A disc is a cushion, not a bone that has 'slipped out'
Between the vertebrae sits a disc with a softer centre and a tougher outer ring. A slip disc means some of that centre has pushed through a weak spot in the ring. Doctors also say herniated, prolapsed, or prolapsed intervertebral disc. The bone itself has not jumped out of place. That is a different problem, called a listhesis.
In the lower back the fragment may press a nerve to the leg. In the neck it may press a nerve to the arm, or, if it is large and central, the spinal cord. The level on the MRI should match the dermatome of your pain. A bulge at a level that does not fit your symptoms may be incidental.
Talk to the clinic about this topic
Leave your name and mobile number if you want the clinic to call about the condition in this article. This is not a diagnosis.
Treatment is stepwise
Unless there is weakness or an emergency, treatment starts outside the operating theatre. Pain relief, a short change in activity, and physiotherapy aimed at confidence with movement are the core. Some people are offered a short course of a nerve-pain medicine. Injections around the nerve are sometimes used for persisting leg or arm pain. They are not a cure of the disc, and they are not right for everyone.
Bed rest beyond a couple of days, traction as a routine, and belts worn all day are less useful than people expect. If you have been told you must never bend again, ask for a second explanation. Most people need to return to normal movement, with a sensible limit on heavy loads while the pain is acute.
When an operation enters the conversation
Surgeons talk about removing the fragment when pain is still severe after a fair trial of the steps above, when a muscle is clearly weak, or immediately when bladder, bowel, or saddle sensation are affected. The operation takes out the piece of disc on the nerve. It does not replace the whole disc in the common form of this surgery.
Cervical disc herniation with cord pressure is a different discussion and should not be treated as a minor arm-pain problem. Bring both the images and the report. Reports sometimes upgrade a mild bulge into alarming language.
